There are over 300 different dermal fillers available world wide. If one filler or type of filler was really better, then most of the other fillers would have disappeared long ago. Every filler available has pros and cons. The ideal filler has yet to be produced. Nevertheless, you can a great result from many fillers. You can also get a terrible result from the same filler. The most important factor is the doctor on the other side of that syringe!! Technique in filling is most likely going to be much more important than which filler is used.

Having said that, most docs I know, including myself, use Hyaluronic Acid Fillers (Restylane, Juvederm, Perlane) more than collagen fillers. The Hyaluronic Acid Fillers are easy to work with, almost painless to inject with good technique, last long enough but not too long, and they can be dissolved with Hyaluronidase.

Generally, patients prefer the results from Hyaluronic Acid-based fillers such as Restylane, Perlane, and Juvederm as they tend to provide more natural looking results and tend to last longer.

Recently, many physicians who treat patients regularly with skin fillers have been using collagen-based fillers less. Fillers which contain hyaluronic acid are more commonly used these days. Examples include Restylane, Juvederm and Perlane. I use hyaluronic acid fillers virtually every day in my office.



Will you look older when you stop using injectables?

Well, that depends:

When we were in our teens and early 20's almost all of us had perfect skin with no wrinkles or lines. As you start to get a little bit older, skin loses its elastic properties, and you start to get the external appearance of aging. In areas where you have very strong muscular activity, you start to see “active” lines. With time, the constant wrinkling of these active lines can “iron in” and make the permanent or static lines of facial expression. These go by many names – crow’s feet, smile lines, bunny lines, nasolabial folds, etc…

If you decide to try out injectables like Botox or Restylane, and only use them for one treatment, you will notice reduction / correction of the line(s) that bother you, and if you stop using the injectable, you will return back to yourself after a period of time (typically 4 months for Botox, and 4 to 8 months for Restylane, depending where you used it.)

However, if you LOVED the effect of the injectable, and used it routinely for 5 or 10 years, and THEN stopped, suddenly you would look much older (5 or 10 years older…) These interventions can cover aging very effectively, and you can maintain your appearance for a number of years before you really need surgical intervention, but you keep aging underneath the cover of the injectable. Your injector will likely have increased / redistributed how the injections were done to keep you looking the way you wanted, but in your mind’s eye, you remember the age of your face when you started using the injectable. So it is a natural reaction to blame the injectable for making you age, but really--they were doing a great job, and it is just time to blame.

The best way to answer the question about "what your face will look like once the effect of Botox and/or Restylane wear off" is to ask you the following question:

How do you like the way you look now? Obviously, if you are content with that look then you do not need anything injected in your face. If you do like the effects of these injectables, then you will have to have them repeated once their effect wears off.

The question you ask is like asking the following: Do I have to keep detailing my car forever once I have it detailed once? The answer is YES if you like the way the car looks when you get it detailed!

Your body is your sanctuary and you need to take care of it any way you deem is best for you: psychologically, spiritually and physically. Don't get swept by the tide because everybody else is getting injections in their face.  From a medical standpoint, once the effects of Botox and Restylane wear off, your face will NOT look older.

Lip augmentation - How painful are filler lip injections?

Juvederm or Restylane to the lips are painful if you do not get adequate anesthesia.  Some people can manage without getting numb but lips are quite sensitive.

I usually do nerve blocks (like the dentists use, they are the best at it) to numb the lips. Once the lip is completely numb, I do the injection.

Next time, ask for nerve block.  But remember that most injectors whether physician, RN or PA are not trained and do not know how to give a nerve block in the mouth.

Botox & Juvederm an excellent combo

Yes. This is an excellent treatment to consider. Please realize though, that there is a small margin of error and flattening of the lip, and certain letters are difficult to pronounce so that singers and wind instrument musicians should not have Botox on the lip.

Tips for keeping your skin young

My skin care routine is quite involved, given my chosen industry, and including both topical and more general recommendations. Again, this will depend on your skin type, but a few rules ring true for everyone:

  • Drink a lot of water
  • Don’t smoke
  • Don’t go to bed with your makeup on
  • Get a light glycolic peel every 4-6 weeks
  • Wear a sunscreen daily, even in winter
  • Get Botox and fillers early and often
  • Use an eye contour cream daily
  • I try not to eat a lot of junk food because when you do it is rare that you look and feel your best.
